And then the designers decided they were overpowered and removed their effectiveness against any enemy you would want to use it against."
Oh, I thought this was going to be about frustrations that occur when designing a game. ...uh. I get really frustrated when new content is added that counters old OP content rather than balancing the old OP content. It tends to make the new content entirely useless except in the case where the old OP content occurs, and the old OP content is still OP for 90% of the total content.
Design frustrations are allowed as well Whenever games have in-game voices that are soft in comparison with the other sound effects so you can't hear what the hell they are saying (FEAR and WoW have their fair share of this)
having to hold a key for several seconds for even the most basic interaction. Annoyingly common in PC ports. Fear 3 and fable 3 are some big offenders.
speaking of that game, the pistol aiming mechanic is horrid. It's actually easier to hit a target out of aiming mode than in it.
An atmospheric game with overly simple achievements causing the damn things to pop up during every moment that should be tense, sad or funny.
